It is when the blisters rupture. You’ll see the sticky fluid oozing from the lesions. Avoid touching the fluid with your fingers because it contains high population of herpes simplex virus. This is the time you need to keep yourself away from anyone.
Your doctor may recommend a prescription antiviral to treat a cold sore. Most antivirals come in a tablet or topical cream form, and some are available in an injectable form. They may be used to reduce the length of an acute outbreak or as a preventative to prevent new outbreaks.
I tried almost all of these remedies and just made my cold sore worst….idk maybe my lips are sensitive I had one a week ago and I left it alone and it didn’t get bad just a lil sore and it was gone. Now I have 2 more I don’t know what to do???
Labial herpes: This is the familiar cold sore that appears on the lip margins (labial refers to the lip). When labial herpes reappears, it usually occurs at the same location each time or a few millimeters away. L-lysine and other vitamin supplements have not been shown to speed healing or reduce outbreaks.
You don’t have to go dump every single thing you own when you get a cold sore, but like your toothbrush, its best to get rid of some things that come in contact with your lips/mouth area-such as lip balm, or make up tools. In addition to this, wash your hands each and every time you touch, or even think you touch, your cold sore. I don’t know why, but doing these 2 things cuts the time the thing lasts down to just a few days. This is true for me, anyway. Oh yea, remember to keep your hands clean until it clears – don’t spread it!
The diagnosis of cold sores is usually based on the appearance of the lesions. Usually, no laboratory test is needed because most sores that look like cold sores are cold sores. Occasionally, mouth sores known as canker sores may be mistaken for cold sores. However, canker sores occur inside the mouth whereas recurrent cold sores usually occur on the lips. If there is a question about the diagnosis, a variety of tests are available, including viral culture and polymerase chain reaction (PCR). To do these tests, a swab is rubbed over an active blister. Swabbing the blister in an attempt to culture the virus in the lab works best in the first 24-48 hours before the blister has crusted over. There is also a way to test for herpes DNA in a swab that has been rubbed on the sore. This type of testing is called a “polymerase chain reaction” or PCR for short. PCR testing is very good at detecting herpes virus, but it is not as readily available as culture.
Putting a whole milk compress on your sore can help speed up the healing, and ease pain. The reason? Milk contains proteins known as immunoglobulins, which are essentially anti-bodies that fight off and prevent viruses-like herpes. It also contains l-lysine. L-lysine helps inhibit the wicked work of an ammino acid called arginine, which has been shown to cause outbreaks, and may help speed up the healing process as well. In short to prevent outbreaks, drink whole milk and get your dose of l-lysine. To help cold sores that have already erupted, make a whole milk compress to soothe the pain and fight off the virus.

I have dealt with “cold sores” my entire life and tried just about everything and ACV is the fastest and most effective treatment. The last time I had one it didn’t even blister just kept putting the ACV on it and in a day there was no sign of it.
Know that cold sores are caused by several variations of the herpes simplex virus (HSV). Despite the name, cold sores aren’t caused by colds. The herpes simplex virus type 1 (HSV-1) is most often the cause. You get it from contact with an infected person’s skin or body fluid. Once contracted, the herpes virus stays with you for life. There is nothing you can do to eliminate the virus, but you can reduce the frequency of outbreaks. 
Primary HSV infection in adolescents frequently manifests as severe pharyngitis with lesions developing on the cheek and gums. Some individuals develop difficulty in swallowing (dysphagia) and swollen lymph nodes (lymphadenopathy). Primary HSV infections in adults often results in pharyngitis similar to that observed in glandular fever (infectious mononucleosis), but gingivostomatitis is less likely.
