Use suppositories, such as those made by Preparation H or Anusol, for 7 to 10 days to relieve irritation and to lubricate the anal canal during bowel movements. Some of these products contain substances that can harm anal tissues if they are used for too long.

Healthy habits can help you prevent hemorrhoids or keep them from getting worse. Eat foods that have lots of fibre, such as fruits, vegetables, and whole grains. Also, drink plenty of water, and get plenty of exercise.

Patients with hemorrhoids may delay seeking treatment because of embarrassment relating to the location. Provide privacy and foster dignity when interacting with these patients. Inform the patient of every step of the procedure. Provide comfort during examination.

Hemorrhoids are not varicosities; they are clusters of vascular tissue (eg, arterioles, venules, arteriolar-venular connections), smooth muscle (eg, Treitz muscle), and connective tissue lined by the normal epithelium of the anal canal. Hemorrhoids are present in utero and persist through normal adult life. Evidence indicates that hemorrhoidal bleeding is arterial and not venous. This evidence is supported by the bright red color and arterial pH of the blood.

In most cases, you can see a general practitioner or your family physician about your hemorrhoid symptoms. If complications arise, you may be referred to a specialist, such as a gastroenterologist or a proctologist.

Stool softeners play a limited role in the treatment of routine hemorrhoidal symptoms. [25] Oral fiber intake and fiber supplements almost always cure constipation and straining. Remember that hemorrhoidal symptoms are due to prolapse, thrombosis, and vascular bleeding; therefore, creams and salves have a small role in treating hemorrhoidal complaints. Suppositories, except for providing lubrication, also have a small role in the treatment hemorrhoidal symptoms.

Apply products that contain medicine to numb an area (local anesthetic). These products often have the suffix “-caine” in the name or the ingredients. Although these products help some people, especially those who have painful external hemorrhoids, some people become allergic to them. Ask your doctor before using these products.

Once you’ve increased the fiber and fluids in your diet, your stools should become softer and pass with less effort. You may help your bowels move even more smoothly by lubricating your anus with a dab of petroleum jelly, says Edmund Leff, MD. Using a cotton swab or just your finger, apply the jelly about 1/2 inch into the rectum.

External hemorrhoids are under the skin around the anus and are therefore visible. Because there are more sensitive nerves in this part of the body, they are normally more painful. Straining when passing a stool may cause them to bleed.

They are caused by straining during bowel movements, sitting for long periods of time, and may also be a secondary symptom of other disorders, such as chronic constipation, obesity, and a high-stress lifestyle.

Over-the-counter ointments and creams, like Preparation H, can be found in almost every drug store and can offer immediate relief. Some can even reduce swelling and help your hemorrhoid to heal faster. If you use a cream with hydrocortisone, though, don’t use it for more than a week at a time.

The rubber band cuts off the hemorrhoid’s blood supply.

External hemorrhoids are far more painful than internal hemorrhoids. They lie within the anus, usually near the surface. Sometimes external prolapse, or enlarge and then protrude. Blood clots can form within these prolapsed external hemorrhoids, making it extremely painful and sensitive to the touch.

Though the term hemorrhoid has received tremendous bad press, truth is that having hemorrhoids is not in itself a dangerous health condition. In fact, hemorrhoids are tissues within the anal canal and nearby areas which help us control bowel movements.1

Doppler-guided ligation of hemorrhoidal arteries (DG-HAL/RAR or THD) has been recently purposed as a safe and relatively painless alternative to hemorrhoidectomy.[14] A special ultrasound device is applied to identify the arteries that supply the hemorrhoids. The surgeon will then will the ligate them, cutting the blood supply and preventing further problems. However, the procedure is associated with a high recurrence rate.[15]

Physical examination is performed to confirm the diagnosis and includes a rectal examination where a finger is used to feel for abnormal lumps or masses. Interestingly, internal hemorrhoids cannot usually be felt. The rectal exam may be deferred if there is intense pain or swelling. In addition, hemorrhoids and constipation may be associated with anal fissures or cracks in the skin surrounding the anus. The associated pain and spasm makes a rectal exam very uncomfortable.

Large internal hemorrhoids and those that do not respond to injection sclerotherapy can be tied off with rubber bands (a procedure called rubber band ligation). The band causes the hemorrhoid to wither and drop off painlessly. One hemorrhoid is treated about every 2 to 3 weeks.

If a painful blood clot (thrombosis) has formed within an external hemorrhoid, your doctor can remove the clot with a simple incision and drainage, which may provide prompt relief. This procedure is most effective if done within 72 hours of developing a clot.

Band it. The most commonly used hemorrhoid procedure in the United States is rubber band ligation, in which a small elastic band is placed around the base of a hemorrhoid (see box above). It takes two to four procedures, done six to eight weeks apart, to completely eliminate the hemorrhoid. Complications, which are rare, include mild pain or tightness (usually relieved with a sitz bath), bleeding, and infection. Other office procedures include laser or infrared coagulation, sclerotherapy, and cryosurgery. They all work on the same principle as rubber band ligation but are not quite as effective in preventing recurrence. Side effects and recurrence vary with the procedure, so consult your physician about what’s best for your situation.

Anorectal varices are common in patients with portal hypertension. [11] Varices occur in the midrectum, at connections between the portal system and the middle and inferior rectal veins. Varices occur more frequently in patients who are noncirrhotic, and they rarely bleed. Treatment is usually directed at the underlying portal hypertension. Emergent control of bleeding can be obtained with suture ligation. Portosystemic shunts and transjugular intrahepatic portosystemic shunts (TIPS) have been used to control hypertension and thus, the bleeding. [12]

Hemorrhoids are diagnosed by a doctor. A visual examination of the anus is usually needed. The doctor may want to perform a rectal examination which will help detect internal hemorrhoids, tumours, polyps, or abscesses.[5] The visual examination usually requires a special device, called anoscope to be inserted in the anus. The procedure is painless unless another disease is presented.

Stapled hemorrhoid surgery, or PPH, was first described in 1997-1998 and has become prominent. [20, 43, 44] This procedure is mainly used to treat internal hemorrhoids that are not amenable to conservative and nonoperative therapies. PPH is suggested for patients with large internal hemorrhoids and minimal external component. This procedure can be done in an outpatient setting with local anesthesia, [45, 46, 47, 48] similar to the protocol used for conventional hemorrhoid surgery. Narcotic use and recovery is significantly decreased compared with conventional operative hemorrhoid surgery.

External hemorrhoids are visible-occurring out side the anus. They are basically skin-covered veins that have ballooned and appear blue. Usually they appear without any symptoms. When inflamed, however, they become red and tender. View hemorrhoid gallery for detailed photos.

Preliminary findings appear to indicate that the novel technique of retroflexed endoscopic monopolar coagulation of grade II-III internal hemorrhoids is safe and effective, and it may be seamlessly incorporated into the end of a colonoscopy for the evaluation of hematochezia. [35] In a involving 100 patients, investigators reported a 5% postprocedure complication rate (3% bleeding, 2% pain; all managed conservatively) and a 6% recurrence rate at a median follow-up of 3 years.

Practice good hygiene by cleaning your anus with warm water during a shower or bath every day. But don’t use soap, as soap can aggravate hemorrhoids. Also avoid using dry or rough toilet paper when you wipe after a bowel movement.

The commonest symptom of internal hemorrhoids is bright red blood in the toilet bowl or on one’s feces or toilet paper. When hemorrhoids remain inside the anus they are almost never painful, but they can prolapse (protrude outside the anus) and become irritated and sore. Sometimes, prolapsed hemorrhoids move back into the anal canal on their own or can be pushed back in, but at other times they remain permanently outside the anus until treated by a doctor.

Hemorrhoids are unique to humans – no other animal develops them. They are very common – up to 86% of people will report they have had hemorrhoids at some time in their life, though people often use this as a catch-all label for any ano-rectal problem including itching. They can occur at any age but are more common between the ages of 45 and 65. Among younger people, they are most common in women who are pregnant.

Coconut oil, which is available online, is a natural moisturizer, which may also help with hemorrhoid symptoms. Applying coconut oil may reduce the irritation and swelling, and it may also help reduce the urge to scratch.

Hot water has a relaxing and anti-inflammatory effect. In order to reduce the discomfort, pain, and swelling of the hemorrhoids, you should put hot water in the bath, sit in it for 15 minutes, and repeat the process every day.

Injection (sclerotherapy). In this procedure, your doctor injects a chemical solution into the hemorrhoid tissue to shrink it. While the injection causes little or no pain, it may be less effective than rubber band ligation.

In an observational study of 402 outpatients who underwent transanal dearterialization for grade II-IV hemorrhoids, complications occurred in 16.7% and the rate of recurrent disease was below 10% for each group. [50]  Bleeding and hemorrhoidal thromboses were the most common complications, with 10 cases each. Of patients with grade II disease (n =16), 6.3% had recurrent disease; grade III (n = 210), 5.8%; and grade IV (n = 176), 9.7%. The investigators indicated advantages of transanal dearterialization include mild pain, fast recovery, low complication rate, and early return to daily activities. [50]

Avoidance of constipation is key when trying to get rid of hemorrhoids. It’s important that you don’t strain while using the bathroom, don’t sit for an excessive period of time and clean yourself thoroughly when finished.

I was a total skeptic but gave it a try with no real hopes that it would work because from what I had read about hemorrhoid treatment scared the heck out of me so it was worth a shot. Establish a history of anal itching, blood on the toilet tissue after a bowel movement, and anorectal pain or discomfort. Ask if the patient has experienced any mucus discharge. Determine if the patient can feel the external hemorrhoids. Elicit a history of risk factors and dietary patterns.

Internal haemorrhoids lie far enough inside the rectum that you can’t see or feel them. They don’t usually hurt because there are few pain-sensing nerves in the rectum. Bleeding may be the only sign that they are there. Sometimes internal haemorrhoids prolapse, or enlarge and protrude outside the anal sphincter. If so, you may be able to see or feel them as moist, pink pads of skin that are pinker than the surrounding area. Prolapsed haemorrhoids may hurt because they become irritated by rubbing from clothing and sitting. They usually recede into the rectum on their own; if they don’t, they can be gently pushed back into place.

After every bowel movement, clean the anal area with a witch hazel pad, a soothing baby wipe or a cotton cloth soaked in warm water. Be thorough but gentle. Aggressive rubbing and scrubbing, especially with soaps or other skin cleansers, can irritate the skin and make your hemorrhoids worse.

Hemorrhoids are swollen blood vessels in the rectum or anus. The hemorrhoidal veins are located in the lowest area of the rectum and the anus. Sometimes they swell so that the vein walls become stretched, thin, and irritated by passing bowel movements. Hemorrhoids are classified into two general categories: internal and external.

A colonoscopy is a procedure whereby a docotor inserts a viewing tube (colonoscope) into the rectum for the purpose of inspecting the colon. Colonoscopy is the best method currently available to diagnose, detect, and treat abnormalities within the colon.

Staples. An alternative to traditional hemorrhoidectomy is called stapled hemorrhoidopexy. This procedure treats bleeding or prolapsed internal hemorrhoids. The surgeon uses a stapling device to anchor the hemorrhoids in their normal position. Like traditional hemorrhoid removal, stapled hemorrhoidopexy is performed under general anesthesia as day surgery.

Working in concert, both sphincters relax and open in order to allow feces and/or gases to exit the rectum. The term “anal canal (c)” refers to the passageway formed by the opening of both sphincters, usually during bowel movement.

Pile pipes should be lubricated with ointment prior to insertion. Suppositories or foams do not have advantages over ointments, creams, and gels.

More aggressive treatment may be needed if a hemorrhoid is thrombosed. Thrombosis is characterized by the pooling of blood in the engorged vessel. It is often bluish in color and may be accompanied by severe and even debilitating pain. If left untreated, thrombosis can sometimes lead to gangrene.
