All things considered, early stage hemorrhoidal disease is the least bothersome outcome of straining — it isn’t unheard of individuals to pass out from severe pain, getting heart attack, or dying from stroke while moving bowels. Despite these considerable risks and lethal outcomes, straining isn’t only considered normal, but is also encouraged by doctors who advise patients to strengthen their abdominal and pelvic muscles, so they can strain even harder.
The term hemorrhoid (singular) means the condition, the sum of symptoms related to the inflammation, enlargement, thrombosis, and/or prolapse of hemorrhoids (tissue bundles). Doctors and patients alike universally refer to hemorrhoidal disease as hemorrhoids, which is technically incorrect. That’s why this chapter refers to hemorrhoidal disease throughout, rather than hemorrhoids.
Stapled hemorrhoid surgery, or PPH, was first described in 1997-1998 and has become prominent. [20, 43, 44] This procedure is mainly used to treat internal hemorrhoids that are not amenable to conservative and nonoperative therapies. PPH is suggested for patients with large internal hemorrhoids and minimal external component. This procedure can be done in an outpatient setting with local anesthesia, [45, 47, 48] similar to the protocol used for conventional hemorrhoid surgery. Narcotic use and recovery is significantly decreased compared with conventional operative hemorrhoid surgery.

Most hemorrhoid cases will benefit from a conservative approach. The aim of treatment is threefold: to alleviate the immediate symptoms, to prevent further exacerbation of the injury, and to resolve the underlying cause.

Sometimes hemorrhoids prolapse, or get bigger and bulge outside the anal sphincter. Then you may be able to see them as moist bumps that are pinker than the surrounding area. And they’re more likely to hurt, often when you poop.
Excisional hemorrhoidectomy is a type of surgery only used in severe cases. It is usually performed under general anesthesia and requires care to avoid damage to the underlying sphincter muscle. While the operation is effective in preventing hemorrhoid recurrence, it can cause significant post-operative pain and usually requires two to four weeks to recover.
If you are older than age 50 or have a family history of colon cancer, it is a good idea to tell your doctor any time you have new rectal bleeding, notice blood on your stools, have changes in bowel habits, or have anal pain.
