Symptoms of herpes usually develop within 2 to 20 days after contact with the virus, although it could take longer. These symptoms may last up to several weeks, varying from one person to the next. In many people, the first infection is so mild that it goes unnoticed. It is possible that a person becomes aware of the “first episode” years after the infection is acquired. Many people who contract HSV are not aware of their infection.

Genital herpes is, therefore, the more commonly transmitted variant of HSV. Since there is no cure for herpes, it is very important for patients and those associating with them to be careful in their interactions, especially during herpes flares.

After an initial genital herpes infection with HSV-2, recurrent symptoms are common but often less severe than the first outbreak. The frequency of outbreaks tends to decrease over time. People infected with HSV-2 may experience sensations of mild tingling or shooting pain in the legs, hips, and buttocks before the occurrence of genital ulcers.

There are two different but closely related viruses that cause genital herpes infection, most commonly the virus associated with genital herpes is herpes simplex virus (HSV) type II. However, HSV type I can also sometimes cause genital herpes. Once someone has been infected by these viruses, there is no way of ever getting rid of them. These viruses belong to a large group of viruses that can hide in a ‘latent’ state in an individual’s body after the first infection with that virus and reactivate at a later stage to cause disease once again.

This can lead to a potentially deadly infection in your baby (called neonatal herpes). It is important that you avoid getting herpes during pregnancy. If you are pregnant and have genital herpes, you may be offered anti-herpes medicine towards the end of your pregnancy. This medicine may reduce your risk of having signs or symptoms of genital herpes at the time of delivery. At the time of delivery, your doctor should carefully examine you for herpes sores. If you have herpes symptoms at delivery, a ‘C-section’ is usually performed.

A doctor will base a presumptive diagnosis on information provided by the patient and on the physical examination. The characteristic appearance of the herpes sores leaves little doubt about the diagnosis, so the typical appearance of the sores is key to the diagnosis. This appearance helps distinguish oral herpes from oral thrush, shingles, gonorrhea, and syphilis. In addition, chapped or sunburned lips can resemble oral herpes, but the tissue stain (Tzanck smear, see below) shows no virus-induced cell changes. Further testing is usually not necessary but is sometimes done.

Some people ask for a blood test to check their prior HSV exposure. We advise that the results may not be accurate enough to assess their risk and that they should, whatever the test result, practice the simple preventative measures mentioned above.

    Oral herpes is a viral infection mainly of the mouth area and lips caused by a specific type of the herpes simplex virus. Oral herpes is also termed HSV-1, type 1 herpes simplex virus, or herpes labialis. The virus causes painful sores on the upper and lower lips, gums, tongue, roof of the mouth, inside the cheeks or nose, and sometimes on the face, chin, and neck. Infrequently, it may cause genital lesions. It also can cause symptoms such as swollen lymph nodes, fever, and muscle aches. People commonly refer to the infection as “cold sores.”
    Some herpes viruses can cause meningitis or encephalitis (inflammation of the brain itself, which is much more serious).
