Unlike HSV2, which usually lies dormant between outbreaks at the base of the spine, HSV1’s latency periods are typically spent in nerve endings near the ear. That is why it is more likely to cause oral herpes than genital herpes.

A major concern with regard to herpes is that it is a communicable infection. It is transmitted through contact with bodily fluids and lesions of the infected person. Genital herpes is, therefore, the more commonly transmitted variant of HSV. Since there is no cure for herpes, it is very important for patients and those associating with them to be careful in their interactions, especially during herpes flares.

Genital herpes is spread by direct contact with an infected person. Sexual intercourse and oral sex are the most common methods of spreading genital herpes. Any type of skin-to-skin contact, however, is capable of spreading herpes.

Other common symptoms include pain, itching, and burning. Less frequent, yet still common, symptoms include discharge from the penis or vagina, fever, headache, muscle pain (myalgia), swollen and enlarged lymph nodes and malaise.[6] Women often experience additional symptoms that include painful urination (dysuria) and cervicitis. Herpetic proctitis (inflammation of the anus and rectum) is common for individuals participating in anal intercourse.[6]

The rash and pain usually subside within three to five weeks, but about one in five people develop a painful condition called postherpetic neuralgia, which is often difficult to manage. In some people, shingles can reactivate presenting as zoster sine herpete: pain radiating along the path of a single spinal nerve (a dermatomal distribution), but without an accompanying rash. This condition may involve complications that affect several levels of the nervous system and cause many cranial neuropathies, polyneuritis, myelitis, or aseptic meningitis. Other serious effects that may occur in some cases include partial facial paralysis (usually temporary), ear damage, or encephalitis.[24] During pregnancy, first infections with VZV, causing chickenpox, may lead to infection of the fetus and complications in the newborn, but chronic infection or reactivation in shingles are not associated with fetal infection.[61][62]

The very first signs of the herpes virus outbreak is the affected area becoming irritated. Most commonly you may feel a tingling or itching sensation around the genitals or the anus, or any other soft tissue area like the mouth or nose. This is your body trying to tell you something; it is a sign that the blisters will come out in this localised area. The skin will become red, start to get itchy and maybe even crack a little. It will feel raw and sore to touch, although you shouldn’t touch it, as it will only create more germs and bacteria in the area.

The virus spreads more quickly when an infected person is experiencing an outbreak. Anywhere from 30 to 95 percent of adults are seropositive for HSV-1, though they may never experience an outbreak. It’s also possible to get genital herpes from HSV-1 if someone who performed oral sex had cold sores during that time.

Acyclovir is an antiviral medication and reduces the pain and the number of lesions in the initial case of genital herpes. Furthermore, it decreases the frequency and severity of recurrent infections. It comes in capsules, tablets, suspension, injection, powder for injection, and ointment. The ointment is used topically and it decreases pain, reduces healing time, and limits the spread of the infection.[13]

Some people will get symptoms within four or five days of coming into contact with the virus. In other people, the virus may be in the body for several weeks, months or possibly years before any appear. When you get symptoms it doesn’t necessarily mean you’ve only just come into contact with the virus.

It is important for pregnant women to be checked for STDs. They can cause women to go into labor too early and may complicate delivery. Many STDs can be passed from mother to baby during pregnancy, childbirth, or after the baby is born. Herpes is diagnosed by taking a sample from an infected area during an outbreak. The herpes virus will usually grow from a swab taken from a ruptured blister. The test will identify the strain (type 1 or type 2) of the herpes virus. PCR (polymerase chain reaction) tests in a pathology laboratory may also be used. Blood tests may assist diagnosis in some cases, but the results can be difficult to interpret.

As long as you’re sexually active, there’s a chance you could get herpes. You’ll make it a lot less likely if you use a latex or polyurethane condom or dental dam every time, for every activity. The dam or condom only protects the area it covers. If you don’t have herpes, you and your partner should get tested for STDs before sex. If you’re both disease-free and aren’t having sex with other people, you should be safe.

Genital herpes symptoms are often mild and infrequent, often going unnoticed. For this reason the majority of people who have genital herpes (sometime referred to as HSV-2) may be unaware they have it. Learning to recognise genital herpes symptoms can help an individual avoid sexual contact during a herpes episode and hence reduce the risk of transmitting genital herpes to a sexual partner.

Unless the immune system is compromised, it suppresses reactivation of the virus and prevents shingles outbreaks. Why this suppression sometimes fails is poorly understood,[37] but shingles is more likely to occur in people whose immune systems are impaired due to aging, immunosuppressive therapy, psychological stress, or other factors.[38][39] Upon reactivation, the virus replicates in neuronal cell bodies, and virions are shed from the cells and carried down the axons to the area of skin innervated by that ganglion. In the skin, the virus causes local inflammation and blistering. The short- and long-term pain caused by shingles outbreaks originates from inflammation of affected nerves due to the widespread growth of the virus in those areas.[40]

Herpes infection of the genital tract is a sexually transmitted infection (sexually transmitted disease or STD). Like in the mouth area, herpes symptoms and signs include a painful, blistering rash around or on the genital or rectal areas. These lesions open and result in painful sores that can take two to four weeks to heal. The sores can sometimes cause painful urination. Recurrent outbreaks are typical, and the time between outbreaks varies among affected people and even within the same individual. Prior to an outbreak, a tingling, burning, or itching sensation may be present on the area of involved skin.

HSV-2. This is the type that commonly causes genital herpes. The virus spreads through sexual contact and skin-to-skin contact. HSV-2 is very common and highly contagious, whether or not you have an open sore.

The study, published in the Journal of the American Medical Association, followed about 500 people between March 1992 and April 2008. They took daily swabs of their genital secretions for 30 to 60 days.
